 This traditional membership offers a comprehensive range of services, though it is not all-inclusive. It includes:

• An annual 60-minute executive physical and four additional visits, which can be used for lab reviews or acute visits tailored to your specific needs. 
• Also included are  two nutrition visits and one InBody scan every three months (additional scans are available for $25 each). 
• Portal messaging is available for up to two messages per month.
• Acute visits cover medical advice or treatment via portal message, phone call, or in-person visit. 
• Medication refills, as long as they are not for new prescriptions, will not be charged as an acute visit. 
• If you're on a controlled medication, a visit every 6 months with vital checks is required, and this will count as one of your visits. These in-person visits are limited to 30 minutes.
• Urgent or emergent on-call care is available for an additional $100 per call. If more extensive services are needed, please refer to our functional tier.

Our weight loss membership includes personalized nutrition support, health coaching, and at least one visit every three months with our provider. Additionally, you'll check in monthly for ongoing accountability and support. We offer customized workout plans, meal plans, and more to help you reach your goals. Each membership also includes one InBody scan every three months to track your progress. Additional InBody scans are available for $25 each. 

• For adults, the membership includes an Executive Physical, urine screening, EKG, lung function screening, and preventative health recommendations, paid annually. 
• For children, the membership includes a sports physical and immunizations (if desired), priced annually. 
• Each family member receives 2 acute visits per year, which can be shared among family members. Additional visits are available for a fixed price per month. 

Please note that this membership does not cover labs or routine care - it’s designed for convenience and to help you avoid unnecessary ER visits. You can also use our urgent/emergent on-call service to count as a visit if a situation can’t wait until the next business day.

If you require more extensive care, you may opt to join our Traditional Membership. This plan does not include medication management, chronic disease management, travel medicine, additional discounts, or laboratory orders.

Patients for this Executive Healthy Child Membership will be working with Brandy, as she has opened up her schedule for 5 additional patients. This membership  includes acute visits, annual physical/sports physical, health planning, specialist/referrals, Immunizations if warranted, etc!

Two Acute visits per person, per year. May divide among family.  Additional visits will be charged a fixed rate. No physicals, labs or routine care. May use urgent/emergent on call to count for one of the visits.  Must be paid annually per person. Call us for price inquiries!  

 Patients will be working with provider Nan O'Brien, who is certified in Women's Health. She will be focusing on perimenopause/menopause, painful/irregular periods, endometriosis, PCOS, insulin resistance, weight loss, birth control needs, fibroids, hair loss, and more. 

Included is:

• An initial hour long consult visit to discuss needs and then 45 minutes for any follow up appointments.
• Each visit will include one follow up portal message and if more messaging is needed, any additional message will be $25.

This is not an all inclusive plan, however more of a pay as you go to work on teens/women's health. It is recommended to keep your primary care provider to manage your acute visits, preventative visits, and any other concerns unless you would like to join one of our other memberships. Insurance can be used to cover laboratory work or use our low cash pay pricing. Specialty testing will be available if warranted at an additional cash pay cost. 

This membership is for individuals interested in utilizing hormone pellet therapy. This is a form of hormone replacement therapy (HRT) where small, rice-sized pellets containing bioidentical hormones are implanted under the skin, usually in the hip or buttock area. These pellets release a consistent, steady dose of hormones over time, typically lasting 4 to 6 months, depending on the individual’s needs.

To get started you will fill out a brief questionnaire to determine your candidacy for the Biote Method. Next, you’ll schedule a lab visit to test your current hormone levels. Patients will schedule a quick follow-up appointment to review their results and, when appropriate, schedule the pellet therapy insertion.
