We believe healthcare should be both accessible and personalized. That’s why HIWFC is embracing a hybrid model, combining traditional insurance billing with a simple monthly membership option. What this means for you, is you can continue using your insurance for eligible medical services, such as sick visits, preventive care, chronic condition management, and medically necessary testing. For patients who want additional support beyond traditional office visits, our membership options offer access to services that help make your healthcare experience more personalized at an affordable rate.
We continue to work with many major insurance plans for medically necessary services. These are typically services that are standard in traditional healthcare settings and are eligible for billing through your insurance provider.
Insurance generally includes:
- Sick visits and treatment for acute illnesses
- Management of chronic conditions (e.g., diabetes, hypertension)
- Preventive care services (annual wellness visits, screenings, immunizations)
- Medically necessary diagnostic testing and procedures
- Referrals to specialists
- Hospital-based care and services outside our office
- Labs, vaccines, procedures
Our membership program is designed to enhance your care experience with more time, access, and convenience. These services are typically not well-covered by insurance or require a more personalized approach.

Instead, we offer our services for a monthly fee. Our approach to your health is to combine both traditional and functional medicine, allowing you to have a holistic view of your health and long-term wellness.

We offer several different membership tiers, and we can help you find the best fit for you after we discuss our tiers with you and once we know your needs. DUTCH testing provides a comprehensive look at your unique hormone patterns, including estrogen, progesterone, testosterone, and cortisol, as well as how your body metabolizes and processes these hormones.

The HIW Foundational Lab Plan is designed to provide a more comprehensive look at your overall health through specialized lab testing and personalized provider support. The plan includes an initial set of foundational labs, and a consultation to review your results and discuss your health goals. Patients also receive discounts on supplements to help support their personalized wellness plan.
Please note: This is a self-pay program and is not billed to insurance. Because these labs are not considered traditional insurance-covered testing, superbills are not available.
With this membership, we work on getting to the root cause of symptoms as well as minimizing medications and supplements, so you feel your best. We can prescribe medication if needed and use band-aids along the way if need be! This is all inclusive including traditional, functional, and weight loss memberships.

This traditional membership offers a comprehensive range of services, though it is not all-inclusive. It includes:
- An annual 60-minute executive physical and four additional visits, which can be used for lab reviews or acute visits tailored to your specific needs.
- Also included are two nutrition visits and one InBody scan every three months (additional scans are available for $25 each).
- Portal messaging is available for up to two messages per month.
- Acute visits cover medical advice or treatment via portal message, phone call, or in-person visit.
- Medication refills, as long as they are not for new prescriptions, will not be charged as an acute visit.
- If you're on a controlled medication, a visit every 6 months with vital checks is required, and this will count as one of your visits. These in-person visits are limited to 30 minutes.
- Urgent or emergent on-call care is available for an additional $100 per call. If more extensive services are needed, please refer to our functional tier.
Our weight loss membership includes personalized nutrition support, health coaching, and at least one visit every three months with our provider. Additionally, you'll check in monthly for ongoing accountability and support. We offer customized workout plans, meal plans, and more to help you reach your goals. Each membership also includes one InBody scan every three months to track your progress. Additional InBody scans are available for $25 each.
We are now offering Direct Patient Care for businesses to help with employee retention, productivity, and lower health care costs! For a low monthly fee per employee, we are able to deliver the best care for your employees.
This DPC membership includes:
- Annual executive physical with EKG (heart screen), lung function baseline, urine screen, and review of preventatives due
- 1-2 traditional laboratory reviews (labs and imaging billed to your insurance or can cash pay or low contracted rate)
- Nutrition/health coach support
- Unlimited same or next day acute visits
- 24/7 on-call for urgent/emergent needs that can’t wait
- 1 In body scan (body composition, BMI, etc.) every 3 months
- Direct staff access via secure portal messages
- Discounts on all Health is Wealth Services like peptide therapy, IV therapy, aesthetics, and so much more!!
- If applicable, we can discuss bringing our services to your office for a one-day onsite wellness day each year!
Memberships do not have a 1 year commitment. For monthly memberships, we ask for a 30 day written notice and for annual memberships, a 30 day written notice before renewal.
Labs are available to be ran through your insurance or at low-cost cash pay that we have contracted with the lab. Any imaging will be used through your insurance. Other services are offered at a discounted rate.
We offer convenient portal access to our patients, providing access to your records, lab results, the ability to message support staff or providers, and more.
Patients will be working with provider Nan O'Brien, who is certified in Women's Health. She will be focusing on perimenopause/menopause, painful/irregular periods, endometriosis, PCOS, insulin resistance, weight loss, birth control needs, fibroids, hair loss, and more.
Included is:
- An initial hour long consult visit to discuss needs and then 45 minutes for any follow up appointments.
- Each visit will include one follow up portal message and if more messaging is needed, any additional message will be $25.
This is not an all inclusive plan, however more of a pay as you go to work on teens/women's health. It is recommended to keep your primary care provider to manage your acute visits, preventative visits, and any other concerns unless you would like to join one of our other memberships. Insurance can be used to cover laboratory work or use our low cash pay pricing. Specialty testing will be available if warranted at an additional cash pay cost.
Hormone pellet therapy is a form of hormone replacement therapy (HRT) where small, rice-sized pellets containing bioidentical hormones are implanted under the skin, usually in the hip or buttock area. These pellets release a consistent, steady dose of hormones over time, typically lasting 4 to 6 months, depending on the individual’s needs.
Bioidentical hormone replacement therapy (BHRT) may offer relief for many common and debilitating symptoms that medical providers often overlook. Some of these symptoms include: Exhaustion, Stress, Foggy thinking, weight gain, sleep disturbances, and more.
Each hormone replacement therapy patient’s symptoms are unique; so is each patient’s path to hormone optimization. Most hormone therapy patients report some relief in as little as four weeks or sooner, but full optimization may take up to six months or more. The bioidentical hormone replacement therapy journey is customized to fit your patient’s specific needs.
The Biote Method of bioidentical hormone replacement therapy (BHRT) uses bioidentical hormones, meaning the chemical structure is the same as the hormones produced by the body, estradiol. Our approach to BHRT using pellets may help avoid unwanted side effects.

To get started you will fill out a brief questionnaire to determine your candidacy for the Biote Method. Next, you’ll schedule a lab visit to test your current hormone levels. Patients will schedule a quick follow-up appointment to review their results and, when appropriate, for pellet therapy insertion.
